The ql201 it's tricky to get. After a lot of time spent the only way to get it is to get it is to have a character capable to roll missions exactly level 201. Roll a mission "find item" for the High Level Omni-Tek/Clan Application Form, give the key to the toon which needs the form and search it.

If you have Shadowland, you can buy the Shadow Level Omni Application Form QL 201 from a Vendor in a garden

My mistake the Shadow Level Omni Application Form requires you to be level 201.
