Ain't used in any quests afaik. Do a search for Leona's Doll.

When it first was introduced, it was needed to enter Steps Of Madness. Dropped from pumpkinheads. After halloween was over, and the pumpkins stopped spawning, the req was removed, and it became a novelty item.

Oh, and the girl near the newland miss terms are looking for a different type of doll, if I remember correctly.
